Online Surveys
We are conducting online surveys to explore how an online personalized health record could be useful to patients, their families and doctors.
We encourage anyone living in Washington State to participate in the surveys.

The surveys are short (around 10 minutes each) and we hope to get input on all of the surveys from the same group of people. We are asking people to provide us with their email addresses so that we can send them a message when each new survey is released. However, you are not required to provide this information. If you do not provide your email, you can still return to this site to take each of the subsequent surveys.
Our pledge: we will not share your email or other personal information with any other party for commercial purposes. We will only use aggregated information to explain the survey results.
