Community Choice Health Record Bank
The Community Choice Health Record Bank (CCHRB) project is organized by Community Choice, a healthcare network based in Cashmere, Washington. It serves a four county area (Chelan, Douglas, Grant and Okanogan Counties) in North Central Washington.

Initially, participants who enroll in the CCHRB will be able to use a personal health record to manage healthcare information for themselves and others who have authorized their access. By participating in this pilot, participants can receive medication history, immunization records and selected diagnostic results from their authorized providers of health care and they can enter information into their own copy of their health record. In future phases of this project, doctors will be able to access the health records in the personal health records, if the patient chooses to give them access.
